Required Medical Form Due April 15th
Dancers must have a medical release and waiver form on file to participate in summer programming. Note that uploading a photo of the front and back of a medical insurance card will be required to complete and submit the medical release and waiver form.
Summer Handbooks & Calendar
Guardians and students are required to review the summer handbook in its entirety upon enrolling. By participating in our programming, you are indicating that you are in agreement with the policies outlined in the handbook linked below. Additionally, please review the summer calendar carefully to confirm arrival and departure dates.
Dormitory Residents
Please refer to the Summer Program calendar to confirm your move in and move out dates before making travel plans. Roommate requests forms are due by April 14.
MOVE IN: 11:00am - 4:00pm | MOVE OUT: By 12:00pm
Airport Transportation
Fees for airport transportation will be applied to your account and charged to the card on file shortly after form is submitted. Unaccompanied minor transportation is only provided to students ages 15 and under. Please review forms thoroughly before booking your flight. Forms are due by June 1.
